Ages 0-5 Enrollment Forms
The following forms must be filled out to enroll your child in the Greene County Early Learning Center.
Children currently ages 2-5 do not have to fill out the two forms marked 'infant only'.
Children enrolled in the infant room (ages 0-2), please fill out all forms, including the two forms marked 'infant only'.
A one-time $25 registration fee per child is due at the time of enrollment. Please call for space availability in our classrooms!
